This is a do-it-yourself packet of documents that are legally enforceable in all states. Simply fill out the forms specifying your wishes, sign the trust document before the required number of witnesses and have it notarized. Have your attorney review and keep your trust on file. Keep a copy with your will and other records.
You fill out the care instructions and forward to us for review. If anything is unclear or we foresee potential problems, well make sure theyre corrected or clarified. Well then forward the standard pet trust document and return copies to you for your attorney, trustee and caregiver. Sign all copies before the required number of witnesses, have it notarized and distribute copies accordingly.
Our most comprehensive level of service includes a one-hour meeting or phone consultation to review your care instructions and trustee and caregiver choices and to resolve any outstanding issues or complications. We then provide you with a customized trust based on this meeting. Services beyond this scope, including acting as your trustee,* are subject to additional fees.
