The Pet Trust Packet
With your trustee’s contact information clearly noted on your emergency wallet card and home emergency information card, you can rest assured that your trustee will be notified immediately if you die or become disabled. He or she will ensure that your pets are transported to your caregiver’s home and that needed funds are distributed as soon as possible. The process is smooth and seamless; your pets are never without guardianship and care. The alternative: without an NB Pet Trust, the fate of your animal companions would be left to chance and/or the goodwill of relatives, neighbors or friends.
The Pet Trust Packet
When you purchase an NB Pet Trust, you will receive a packet containing the following documents:
Animal Companion Information Sheet(s). Here is where you provide all of the pertinent information about your pets, including likes and dislikes, special food, etc.
Emergency Pet Care Information Sheet. This is a quick reference, and contains trustee, caregiver and veterinarian contact information, as well as descriptions and special needs of all of your animal companions.
Establishing an NB Pet Trust. A basic overview of steps you need to take to implement your NB Pet Trust.
Estimate of Pet Care Costs. A worksheet enabling you to calculate each pet’s monthly and yearly costs.
Selecting a Caregiver. Tips for choosing the person who will provide daily care for your pet according to your detailed instructions.
Selecting a Trustee. Tips for choosing a trustee who will oversee your pets’ care and administer the trust funds.
Power of Attorney. Legal document transferring power of attorney to your designated representative.
Refrigerator Alert Magnet. A magnet that lets visitors know how many pets reside in the house, so each animal can be found and cared for in the event something happens to you. Also includes emergency caregiver‘s contact information.
Wallet and Keychain Alert Cards. Wallet card contains your address, the number of animals in your home, your trustee’s and emergency caregiver’s contact information and the location of your pet trust documents. Keychain card has emergency caregiver’s name and contact information.
Pet Trust Document. If you purchase our Silver Trust you will receive the actual pet trust document in your packet. If you choose Gold or Platinum level services, you will receive the trust in the mail after we have reviewed your information, and in the case of the Platinum Trust, met with you to discuss your personal situation.
